In 2025, St. Marks Place in the East Village still carries that familiar edge.

Every generation brings its own version of the outsider—people pushing back against the establishment, against the grind, against ambition itself. They resist productivity, often turning to drugs, alcohol, and other ways to escape the harder parts of life.

You see it in the punk and metal gear—costumes that speak to rebellion, anger, disillusionment, and a deep distrust of authority.

But underneath that is something more raw. Many of these lives are shaped by dysfunction, broken homes, trauma. The performance of rebellion often masks a personal history that’s much less theatrical.

As a psychiatrist who has worked with the homeless on the streets of Manhattan, the mentally ill can be taken against their will to an ermergency room only if they present a danger to self or others or are unable to care for themself showing demonstrative signs of physical deterioriation or very bizarre behavior. Police can assist with this though they are never happy doing it.

Those using drugs that exacerbate their mental illness are the most difficult to treat and communicate with.

When a person is sent to an ER and given medication those that clear and deny any suciidal or homicidal ideation will be discharged. If someone doesnt clear they can be held 72 hours on an inpatient unit if 2 MDs attest that they need treatment. Most who know the game and no longer are deemed a danger will agree to an out patient appointment accept a shelter referral and a script for meds but will soon disappear back to the streets.
Thats the way it is......................

A heroin nod happens when a person is drifting in and out of consciousness.

Someone who is experiencing heroin nods might appear as if they are falling asleep with their eyes closing and head drooping. Then, they may suddenly wake back up, only to return to their previous state of appearing to fall asleep. This can happen no matter the position the person is in, whether standing, sitting, or lying down.
